Lipoedema surgery with German statutory health insurance: consultation in Düsseldorf, operation in Wertheim

Consultation room

With health insurance, your path is split between two sites of the group:

  • Consultation and examination: Klinik am Seestern, Düsseldorf
  • Preparation and planning: Klinik am Seestern, Düsseldorf
  • Operation: Nationalklinik of the Weight Doctors, Wertheim
  • Aftercare and check-ups: Klinik am Seestern, Düsseldorf

As a self-pay patient, by contrast, you are operated on at Klinik am Seestern in Düsseldorf itself; more on this under Lipoedema surgery as a self-pay patient.

Many women with lipoedema have worn compression garments and attended lymphatic drainage for years before the question of an operation arises. Since health insurance can cover liposuction in all stages, a procedure is within reach for significantly more people affected. Today the hurdles lie mainly in the requirements, which must be met and documented.

At Klinik am Seestern in Düsseldorf, you book your consultation on lipoedema surgery directly as a patient with German statutory health insurance, as a self-pay patient or still undecided. In the consultation we examine you, check together with you which requirements are already met and what is still missing, and plan the next steps.

You will find us on the left bank of the Rhine in the Lörick district, in the Am Seestern office district not far from the B7. From the main station, the U77 takes you without changing to the Am Seestern stop; if you come by car, you can park free of charge on the clinic grounds. Whether you come from Düsseldorf itself, from Meerbusch or from Neuss: appointments before and after a treatment remain easy to reach too.

Since October 2025: liposuction for lipoedema covered by health insurance

On 17 July 2025, the Gemeinsamer Bundesausschuss (G-BA, Federal Joint Committee) decided that liposuction for lipoedema in all stages becomes a service of statutory health insurance. The decision has been in force since the beginning of October 2025. Until then, cost coverage was limited to stage 3.

The decisions of the G-BA apply to all statutory health insurance funds. Whether you are insured with Techniker Krankenkasse, an AOK, Barmer, DAK or a company health insurance fund makes no difference to the rules. You no longer need an individual application to your insurer: the treating doctors check and document whether the requirements are met.

The health insurance requirements at a glance

For health insurance to be able to cover lipoedema surgery, all of these points must apply:

  • Confirmed diagnosis by a specialist doctor authorised for this
  • At least 6 months of conservative therapy without sufficient improvement
  • No weight gain in these 6 months
  • BMI no higher than 35, between 32 and 35 with a suitable WHtR
  • Diagnosis and the decision for surgery by different doctors

The diagnosis for the insurer is made by specialists in internal medicine and angiology, in dermatology, in physical and rehabilitative medicine, or by doctors with additional training in phlebology. How the examination works, you can read under Lipoedema diagnosis in Düsseldorf. What belongs to conservative therapy is explained under Treating lipoedema without surgery.

BMI and WHtR: What counts for your lipoedema surgery under health insurance

BMI alone does not decide, but it sets the framework: if it is between 32 and 35, waist circumference in relation to height also counts.

You can calculate your BMI and your WHtR with our WHtR calculator. The calculator also shows which limit applies for your age. The binding assessment is made in the medical examination.

BMI and WHtR

What BMI means for the health insurance benefit

Three BMI ranges determine whether it stands in the way of lipoedema surgery under health insurance.

Scale in three sections, the pointer is in the first section

01BMI under 32no obstacle

BMI does not stand in the way of health insurance coverage. The other requirements count: diagnosis, conservative therapy and a stable weight.

02BMI 32 to 35WHtR also counts

Here the waist-to-height ratio (WHtR) also plays a part in the decision. It must not exceed the limit for your age: 0.50 up to age 40, then 0.01 more per year of age, 0.60 from age 50. At 45, the limit is therefore 0.55.

03BMI over 35weight first

Liposuction covered by health insurance is then not possible at first. A treatment of overweight comes first, on which we also advise you under Weight loss.

Stage 1, 2 or 3: What health insurance covers today

Since October 2025, the stage no longer decides whether health insurance pays. That fits the current guideline: the stages describe how the skin and fatty tissue look, not how severe the symptoms are. Whether liposuction makes sense depends on your symptoms and on how they respond to conservative treatment. What the individual stages describe, you can read under Lipoedema symptoms and stages.

Your path to lipoedema surgery with health insurance in Düsseldorf

  1. Step 1

    First assessment

Consultation at Klinik am Seestern. We examine you, talk about your symptoms and previous treatments, and show you which requirements are already met.

  2. Step 2

    Confirmed diagnosis

Specialist findings. A specialist doctor authorised for this confirms the diagnosis for the insurer. We tell you who qualifies for this.

  3. Step 3

    At least 6 months

Conservative therapy. Compression, lymphatic drainage and exercise, prescribed by a doctor and documented. Your weight should not increase during this time.

  4. Step 4

    Second medical review

Decision on surgery. If the symptoms persist, a different doctor from the one who made the diagnosis checks whether liposuction is indicated.

  5. Step 5

    Operation in Wertheim

The procedure. Together we plan which body regions will be treated and whether several sessions make sense. Under health insurance, operations are usually performed at the Nationalklinik of the Weight Doctors in Wertheim.

  6. Step 6

    Afterwards

Aftercare in Düsseldorf. For check-ups and aftercare, you come back to Klinik am Seestern.

These documents help with health insurance coverage

  • Findings report with the confirmed diagnosis
  • Prescriptions for compression and lymphatic drainage
  • Proof of at least 6 months of conservative therapy
  • Your weight record over this period
  • Height, weight and waist circumference for BMI and WHtR
  • Earlier doctors’ letters and findings

Bring what you already have to your consultation in Düsseldorf. We will discuss with you what is still missing.

What health insurance usually does not cover

The health insurance benefit applies to liposuction for lipoedema. Skin tightening afterwards, for example on the thighs or upper arms, is usually not covered by health insurance; more on this under Skin tightening after lipoedema surgery. Procedures for purely aesthetic reasons also remain self-pay services.

Privately insured or entitled to Beihilfe (German civil service allowance)?

Private health insurance providers decide according to your tariff, Beihilfe offices according to their own regulations, and for female state civil servants in North Rhine-Westphalia, for example, according to the state’s rules. Clarify before treatment which documents your insurer or Beihilfe office requires. The proofs mentioned above are also helpful there.

Self-pay patient or still undecided?

Not every patient meets all the requirements or wants to wait out the period of prior treatment; what applies then is explained under Lipoedema surgery as a self-pay patient. As a self-pay patient, you are operated on at Klinik am Seestern in Düsseldorf itself, without the requirements of health insurance, but not without an examination: whether lipoedema surgery is an option for you, we decide together. After the consultation you receive a personal treatment quote; financing is possible through our financing partner. You can read more on costs and health insurance under Costs, health insurance and financing.

Frequently asked questions

Does German statutory health insurance pay for lipoedema surgery in Düsseldorf?

Yes, that is possible. Since the beginning of October 2025, statutory health insurance can cover liposuction for lipoedema in all stages if the requirements of the Gemeinsamer Bundesausschuss (Federal Joint Committee) are met. The operation then usually takes place at the Nationalklinik of the Weight Doctors in Wertheim; you attend consultation, preparation and aftercare at Klinik am Seestern in Düsseldorf.

Do the rules apply equally to TK, AOK, Barmer and DAK?

Yes. The decisions of the Gemeinsamer Bundesausschuss (Federal Joint Committee) apply to all statutory health insurance funds, whether Techniker Krankenkasse, AOK, Barmer, DAK or a company or guild health insurance fund. What matters is your medical requirements, not the name of your insurer.

Do I have to submit an application to my health insurance?

Since October 2025, you no longer need an individual application. The treating doctors check and document whether the requirements are met. Your documents on diagnosis, therapy and weight are still important for this.

Which BMI applies to lipoedema surgery under health insurance?

BMI may be 35 at most. Below 32, it does not stand in the way of health insurance coverage. Between 32 and 35, the waist-to-height ratio (WHtR) must not exceed the limit for your age, 0.50 up to age 40, 0.60 from age 50. Your value is calculated by our WHtR calculator.

Does health insurance also pay for lipoedema stage 1 or 2?

Yes. Until October 2025, cost coverage was limited to stage 3; since then it has applied regardless of the stage. Whether an operation is an option depends on your symptoms and the other requirements.

What counts as conservative therapy?

This mainly includes compression, manual lymphatic drainage and exercise. It should be prescribed by a doctor, last at least 6 months and be well documented. More on this under Treating lipoedema without surgery.

Does health insurance also cover skin tightening after lipoedema surgery?

Usually not. Tightening of the thighs or upper arms after liposuction is mostly a self-pay service. More on this under Skin tightening after lipoedema surgery.

What applies to privately insured people and female civil servants with Beihilfe?

Private health insurance providers and Beihilfe offices decide according to their own rules and tariffs. Ask before treatment which documents are required. A findings report and proof of prior treatment help there too.

What does lipoedema surgery in Düsseldorf cost if health insurance does not pay?

Without health insurance coverage, you pay for the treatment yourself. The price depends on which body regions are treated and how much tissue is removed by liposuction. After the consultation you receive a quote; you can finance it through our financing partner.

Where does lipoedema surgery under health insurance take place?

Usually at the Nationalklinik of the Weight Doctors in Wertheim, as with a stomach reduction under health insurance. Consultation, preparation and aftercare remain in Düsseldorf at Klinik am Seestern. As a self-pay patient, you are operated on at Klinik am Seestern itself.

Where in Düsseldorf can I get advice as a patient with German statutory health insurance?

At the Klinik am Seestern, Am Seestern 4 in 40547 Düsseldorf-Lörick. The clinic is on the left bank of the Rhine near the B7, the U77 runs directly from the main station to the Am Seestern stop, and there is free parking on the grounds.
